THE RELEVANT RADIO® PHOTO RELEASE FORM

I hereby grant Relevant Radio, Inc. permission to use the likeness of my child(ren) or grandchild(ren) in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.

I understand and agree that all photos will become the property of Relevant Radio and will not be returned. I further understand that my name and/or the first name(s) of my child(ren) or grandchild(ren) may be included in captions associated with the photo(s).

I hereby irrevocably authorize Relevant Radio to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein the likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

I hereby hold harmless, release, and forever discharge Relevant Radio, along with its legal representatives, licensees, and assigns, from any and all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate may have by reason of this authorization, including without limitation any and all claims for libel and/or invasion of privacy.

I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I ACCEPT THE TERMS OUTLINED ABOVE AND AFFIRM THAT I AM THE PARENT OR LEGAL GUARDIAN OF THE CHILD(REN) OR GRANDCHILD(REN) IN THE PHOTO AND THAT I AM AT LEAST 18 YEARS OF AGE.
